Community Transition

Community Transition is one of the 5 Core tenets of our work at Disability Network Mid-Michigan that builds community and changes lives.

We believe that everyone has the right to determine the kind of life they would like to lead while having opportunities to become true and contributing members of the community.

Youth Transition

Students will participate in skills-based workshops or 1:1 activities designed to assist with mastering basic skills needed to become independent adults. Students are offered a variety of experiences geared towards reaching independence. These services include pre-employment skills, self-advocacy, career exploration, and customized independent living skills.

DNMM coordinates its services with students in a school setting in one of two ways:

DNMM has coordinated with faculty at your student’s school to provide skills-based services. In most cases, your student’s teacher has requested these services on a 1:1 basis or for the full class.
The student is currently working with Michigan Rehabilitation Services. Their counselor has authorized DNMM to complete employment-related skill-building with your student.

Students receiving this service also have access to our Skills Site. The Skills Site is a fully functional apartment that we rent to work on Independent Living Skill building. Activities at the Skills Site can include learning how to complete a load of laundry, practicing cooking skills, proper cleaning, hygiene assistance, money management, and more.

Students are offered the opportunity to develop an Independent Living Plan (ILP) which encourages goal setting and is an important skill for success later in life. Students will have the opportunity to participate in community-based activities with our staff during the school day. This could include visiting possible places of employment and learning how to fill out job applications, taking a trip to the bank to learn about their different services, getting a state ID card, learning how to grocery shop, and more. Whether at the Skills Cite or in the community, your student will always be accompanied by a trained DNMM employee.

STAGES Youth Group visiting the Zoo

STAGES Summer Program - Starts June 25

Participants are young people with disabilities who learn to live and work more independently.

STAGES = “Students in Transition Achieving their Goals Empowered and Self-confident”

Activities take place from mid-June to mid-August

Activities include:

Weekly hangout with peers
Health care, wellness and exercise
Employment and business tours
Fun Fridays that include activities such as kayaking, biking, a trip to the zoo, etc.

Nursing Facility Transition

We believe people faced with decisions about long-term care should know their options. Through our Nursing Facility Transition Program Disability Network Mid-Michigan provides individuals, regardless of their ability, an opportunity to choose community-based living as a viable alternative to nursing facility residency. The program is open to anyone who resides in a nursing facility, receives Medicaid, and requests to return to their own home or to a more independent living environment. Each situation is based on individual needs and preferences.
